The Children's Medical Services (CMS) Branch of the California Department
of Health Services provides a comprehensive system of health care for children through preventive screening, diagnostic, treatment,
rehabilitation, and follow-up services. CMS integrated the Child Health and Disability Prevention Program (CHDP) and the California
Children Services (CCS) at the state level. The two programs are merged in some counties and separate in others.

CHILD HEALTH AND DISABILITY PREVENTION PROGRAM (CHDP)
- A program which offers regular, preventive health assessments
to eligible children who may or may not have obvious health problems.
- Potential health problems which are found early may be
prevented, corrected, or treated earlier, thus reducing their efforts on both the child and his/her family.
- CHDP works with private physicians, local health departments,
schools, nurse practitioners, dentist, health educators, nutritionist, community clinics, nonprofit health agencies, and social/community
service agencies.
CHDP PROGRAM SERVICES:
- Health and developmental history
- Physical and dental examinations and nutritional
assessment.
- Immunizations
- Vision, hearing, tuberculin, and lead testing
- Some laboratory test (e.g. sickle cell, urinalysis,
hemoglobin/hematrocrit, pap smears)
- Age-appropriate health education and anticipatory
guidance

ELIGIBILITY FOR CHDP PROGRAM HEALTH ASSESSMENTS:
- CHDP provides periodic preventive health services to
Medi-Cal beneficiaries under the regulations of the federal Early and Periodic Screening, Diagnosis, and Treatment (EPSDT)
Program.
- All California Medi-Cal recipients from birth
to age 21 and children currently enrolled in Head Start and State Preschool Programs are eligible for health assessments based
on the following schedule:
- under 1 month 10-12 months
- 1-2 months 13-15 months
- 3-4 months 16-18 months
- 5-6 months 2 years
- 7-9 months 3 years
- 4-5 years 6-8 years
- 9-12 years 13-16 years
- 17-20 years
A health check-up will be given, when required, for foster care, sports, or camp. Check
with your local CHDP program.
- CHDP also provides periodic preventive health services
to low income eligible (200%ers) children from birth through 18 years of age. The periodicity schedule above applies through
18 years.
Your child may qualify for continued health benefits.
CHDP PROGRAM SCHOOL
ENTRY REQUIREMENTS:
- By law, all children entering the first grade
are required to have either a certificate of CHDP health examination, proof of recent assessment, or a waiver on file at the
school in which they enroll.
- The CHDP program works with the Department of
Education administrating and monitoring this requirement and assist low-income children to meet the requirements by providing
them with state-paid health assessments.
