Client fills out Application for Permit and has Percolation Test performed.
Brings back both for approval.
4.
Permit fee is collected by Environmental Health & receipt is given to party paying.

Sanitarian will make a site evaluation for approval of location and Percolation Test.

Sanitarian will issue Septic Permit if septic area is approved.
