**All medical services are to be provided to the child by a CCS paneled
provider.
"Paneled Provider"
means an individual who has been determined by the CCS program to meet the advanced education, training, and/or experience
requirements for his/her provider type in order to render services to a CCS applicant or client. For a list of CCS panel providers

How do I find out
if my child has an approved CCS medically eligible condition?
Only certain
conditions are covered by CCS. For more information on medical
eligibility go to: medical eligibility link at the bottom of the page.*Anyone
may refer a child to CCS-public health/school nurses, Health Families (HF) Plans, Medi-Cal Managed Care Plans, other agencies,
or the family itself to determine if the child has an approved CCS medically eligible condition. Most children are referred
by the family physician, specialist, or hospital.
What
Must a Family Do To Obtain CCS Services?
Families (or the applicant if age 18 or
older, or an emancipated minor) must:
- complete
an application form (in English or Spanish) and return it to their county CCS office;
- give
CCS all of the information requested so CCS can determine if the family qualifies;
- apply to Medi-Cal
